Request Release of Medical Information

The information contained in a patient’s medical record is confidential. It is a legal requirement that Evangelical Community Hospital receive specific authorization before releasing this information.

All requests for information are completed electronically. The Hospital does not process releases, under any circumstances, for pick up at the Hospital. When placing a request, please allow appropriate time for processing and delivery.

Release of Information

Please note: You will be redirected to a third-party system managed by MRO. Evangelical Community Hospital works with the release of information vendor, MRO, to coordinate providing copies of medical records to patients and authorized representatives. The MRO portal has a different privacy policy and may have different security than evanhospital.com.
