Choose a provider that you believe will work best for your schedule in terms of appointment times, location, and your fluent language. Being able to communicate fully with your provider will ensure you're able to get the best care possible.

What Does my Provider Care Provider Need to Know?
Family History
Make sure to ask your family members if there are any medical issues that run in your family, from heart issues, diabetes, blood pressure issues, and more! Your PCP will check if any of these medical issues have passed onto you.
Vital Signs, Height, and Weight
Your PCP will check your vitals at each appointment to establish your normal range. This will signal when a potential issue arises that deviates from your average vitals.
Sexual Orientation and Gender Identity
While you may choose not to disclose your sexual orientation or gender identity, any information provided will be kept confidential by law, and our provider will use it to better understand and meet your healthcare needs. Your provider is a safe space for you to be honest about any chemicals you put in your body. This helps them to be on the look out for potential issues or negative interactions with medication.
Safety at Home
You will be asked a series of questions to determine if you are in a safe environment at home. Your provider can guide you to protective services if necessary.

Your provider will go ask if you have ever felt depressed, contemplated harming yourself, or are otherwise suffering from mental health issues. Being honest with your provider will help them get you the correct diagnosis and treatment to be your best self!
